要旨
  Basedow病病態研究の歴史を振り返ると,long acting thyroid stimulator(LATS)の発見以来約65年が経過する。その間,抗甲状腺刺激ホルモン受容体(TSHR)自己抗体発見,各種抗TSHR抗体測定法開発,TSHR cDNAクローニング,Basedow病/眼症動物モデル作出,単クローン性抗TSHR抗体単離,TSHR細胞外領域蛋白発現などの研究成果が積み上げられてきて,現在がある。本稿ではこれらのうち,歴史に簡単に触れた後,刺激型および阻害型抗TSHR抗体について概説する。4つのグループ─Vassartら,Daviesら,Bangaら,Rapoportら─は遺伝子免疫を施した免疫動物から単クローン性抗TSHR抗体を単離した。刺激型から阻害型,ニュートラル型まで種々の性質・エピトープを有する抗体が得られた。一方でRees SmithらはヒトBasedow病患者リンパ球からの刺激型および阻害型の単クローン性抗TSHR抗体単離に成功した。実験的に得られた抗体に比較して,ヒトでの限られたimmuno-dominant(免疫優勢)領域のみを認識するため,ほぼすべてのヒトBasedow病抗体と結合競合するので,非常に有用であり,実際既に抗体測定キットに使用されている。ヒトのBasedow病患者甲状腺機能はTSAb/TBAb比で説明しうると考える。最後に今後抗体療法にも期待がもたれる。

Summery
When looking back in history of Graves’ disease, it has been nearly 60 years since the discovery of long acting thyroid stimulator(LATS). During this time period, various research achievements such as the discovery of anti-TSH receptor(TSHR) autoantibody, establishment of assay methods to measure anti-TSHR antibody titers, the cloning of TSHR cDNA, generation of Graves’ hyperthyroidism/ophthalmopathy, isolations of monoclonal anti-TSHR antibodies and production of the TSHR extracellular domain protein have been accumulated. This review article, after glancing at history of Graves’ disease, summarizes stimulating and blocking anti-TSHR antibodies.Four groups including Vassart et al., Davies et al., Banga et al. and Rapoport et al. succeeded for isolation of monoclonal anti-TSHR antibodies from animals received genetic immunization. Various different types of antibodies from stimulating type to blocking to neutral with distinct epitopes were obtained. On the other hand,Rees Smith et al. isolated stimulatory and blocking monoclonal antibodies from Graves’ patients. These antibodies,unlike those obtained experimentally, recognize the immune-dominant region on the TSHR, therefore competing with most of patients’ antibodies for TSHR binding, and proved to be very useful for TSHR assay kits. Thyroid function in Graves’ patients is thought to be explained by TSAb/TBab ratios. Finally, much is also expected of future antibody therapy.

AITD患者血清におけるApparentニュートラル型TSH受容体抗体について

要旨
  刺激型のTRAbにより原発性甲状腺機能亢進症が引き起こされるBasedow病は,橋本病とともにAITDと包括され,両者の患者血清からはTgAbやTPOAbが高頻度に検出される。このことから,これらの疾患では甲状腺細胞のさまざまな構成蛋白質に対して網羅的に抗体が産生されており,Basedow病におけるTRAbはTSH受容体のTSH結合領域に対し選択的に産生された抗体であるが,その部位だけを狙って産生されたと考えるよりは,TSH受容体の細胞外ドメインに対してあまねく産生された抗体のうち,たまたまTSH結合領域に対してできた抗体が病因になったと考えるほうが自然である。実際,Basedow病の患者血清にはTSH受容体細胞外ドメインの他の部位に対しても抗体が産生されていることが示されている。ただ,現在臨床応用されているTBIIやTSAbの検査キットはTSH受容体のTSH結合領域に対してできた抗体(=TRAb)だけを検出する。本稿では,最近経験した,TBIIが強陽性にもかかわらず刺激活性(TSAb)も阻害活性(TBAb)も示さない,いわば,apparentニュートラル型TRAbというべき自己抗体を有する甲状腺機能低下症の症例を提示し,その特徴について議論した。

Summery
Graves’ disease(GD) caused by autoantibodies to the thyrotropin receptor(TRAb) is classified as the autoimmune thyroid diseases(AITD) together with Hashimoto thyroiditis(HT), and anti-thyroglobulin autoantibodies(TgAb)and/or anti-thyroid peroxidase autoantibodies(TPOAb)are frequently detected in the sera from patients with AITD. These findings suggest that various autoantibodies are comprehensively produced against the configuration factors of thyroid gland. Alternatively, although TRAbs are the autoantibodies selectively generated against the TSH-binding domain of TSH receptor, it is reasonable to consider that the patients incidentally obtained TRAbs develop GD among the patients with AITD, rather than that the patients with GD exclusively produce the autoantibodies against the TSH-binding domain. In fact, the antibodies against the hinge region of ectodomain of the TSH receptor, referred to the neutral TSH receptor autoantibodies, are detected in the sera from some GD patients. However, they do not basically affect TSH-binding to the TSH receptor, indicating that their thyroid-binding inhibitory immunoglobulins(TBII) are negative. Here, we present a rare case whose autoantibodies are strong positive for TBII but having neutral(neither stimulating nor blocking) biological activities to the TSH receptor. I propose such autoantibodies as ‘apparent’ neutral TRAb.

[簡潔表題]甲状腺分化癌に対する分子標的薬治療の予定休薬法

要旨
根治切除不能な甲状腺分化癌の分子標的薬治療中には多彩な有害事象(AE)が出現し減量や休薬を要す。しかしAEが顕在化し日常生活に制限を来してからの減量や休薬は患者の治療意欲を低下させうる。一方で過度の減量は治療強度を維持できなくなる恐れがある。われわれはレンバチニブ(lenvatinib)長期投与に伴うAEの回避と軽減を目的に,2015年7月~ 2017年10月までに当院で投与開始された20例のうち5例に患者の同意を得て定期的な予定休薬を試みた。中央値26.7ヵ月(23.8 ~ 32.4ヵ月)の観察期間においてAEを回避・軽減しつつ,部分奏効あるいは不変を維持した。生命予後が比較的良好な甲状腺分化癌では治療期間が長期となるため,AEを回避・軽減することでQuality of Life(QoL)維持に配慮した治療法の選択が重要である。

[Running head] Molecular-targeted treatment of differentiated thyroid cancer with periodic drug withholding

Molecular-targeted agents are a new class of agents for treating radioiodine-refractory differentiated thyroid cancer.As adverse events are often observed in patients undergoing molecular-targeted therapy, it frequently becomes necessary to lower dosages or to discontinue medication. However, this may lead to the risk to reduce doses too low for effective treatment. In addition, lowering dosages or discontinuing medication due to adverse side effects negatively affects patients’ quality of life (QOL) and could weaken their desire to continue treatment. Aiming to avert and decrease adverse events resulting from long-term intake of lenvatinib, we modified oral administration schedules of lenvatinib to include periodic off -days in 5 of 20 patients with informed consent who’s therapy was initiated at our hospital between July 2015 and October 2017. All 5 patients were able to preserve their QOL without signs of disease deterioration in the observation period which a median of 26.7 months(23.8-32.4 months). Due to the relatively favorable prognosis of differentiated thyroid cancer, treatment with moleculartargeted agents is administered over the long term; therefore, this long duration of treatment makes it important to select treatment plans that take into consideration the patient’s QOL.
