要旨
 近年,世界的に甲状腺癌の罹患率が増加しており,特に,サイズが小さい甲状腺癌の発見が増えていると報告されている。そのため,甲状腺結節の診断においては,超音波検査によりリスク状態を細胞診実施前に正確に把握し,診断方針を決定することが重要となっている。日本では,日本超音波医学会(JSUM)の「甲状腺結節(腫瘤)超音波診断基準」が広く使用されており,その内容は最近のメタアナリシスや海外のガイドラインで示されている超音波所見と同様である。また,低リスク甲状腺癌に対応するため,日本乳腺甲状腺超音波医学会(JABTS)および日本甲状腺学会(JTA)により,穿刺吸引細胞診(FNAC)実施基準が公表され,諸外国からも近年,FNACの実施基準が発表されている。これらのガイドラインはいずれも結節の大きさと超音波所見に基づくものである。日本のガイドラインを諸外国のガイドラインと比較すると,1cm以下の結節に対する基準において,ATAおよびETAの基準はほぼ同様の基準であった。

[Running head]Diagnosis of thyroid(micro)nodules

In recent years, the incidence of thyroid cancer has been increasing worldwide, and in particular, the detection of thyroid cancers of small size has been reported to be increasing. Therefore, in the diagnosis of thyroid nodules, it is important to accurately determine the risk status with ultrasonography and the diagnostic strategy before fine needle aspiration cytology(FNAC)is performed. In Japan, the Ultrasound Diagnostic Criteria for Thyroid Nodule(3rd edition)of the Japan Society of Ultrasonics in Medicine is widely used, and its ultrasonographic features are similar to those of shown in recent meta-analyses and overseas guidelines. In addition, to cope with low-risk thyroid cancer, the Japan Association of Breast and Thyroid Sonology and the Japan Thyroid Association have published criteria for the implementation of FNAC, and other countries have also published similar criteria in recent years. These are based on nodule size and ultrasonographic findings. Comparing the Japanese criteria with those of other countries, the ATA and ETA criteria are almost similar even in the criteria for nodules smaller than 1 cm

 病理医にとって,「過剰診断」は誤診の一つであり,良性病変を誤って悪性病変と判断することを意味するが,ここでは,疫学的に使用されている「過剰診断」に対する病理学的対応について述べる。「過剰診断」の抑制には,発見の抑制,診断の抑制,治療の抑制の3つがあり,病理学的な対応としては診断の抑制が相当する。具体的には,境界悪性群の採用,細胞診報告様式の変更,穿刺材料を用いた遺伝子検査の導入である。

[Running head]Overdiagnosis and Pathology

For pathologists, “overdiagnosis” is a form of misdiagnosis, meaning that a benign lesion is erroneously determined to be a malignant lesion. Here, however, the pathologist’s response to “overdiagnosis” in the epidemiological sense is described. There are three ways to reduce “overdiagnosis”: to reduce detection, to reduce diagnosis, and to reduce treatment. The second one is the pathological response, specifically, the adoption of borderline malignancy, revision of cytology reporting system, and the introduction of molecular testing using aspirated materials.

 1993年に隈病院ではじまった成人の低リスク微小乳頭癌(以下,微小癌)のアクティブ・サーベイランスは,その後,日本内分泌外科学会・日本甲状腺外科学会発行の『甲状腺腫瘍診療ガイドライン』や,アメリカ甲状腺学会(ATA)のガイドラインに採用された。さらに,2021年に日本内分泌外科学会からconsensus statementsが,そして日本甲状腺学会からposition paperが出版されるに至った。このマネジメントはきちんと症例を選べば安全に行え,患者のQOL向上に大いに寄与するものである。  本稿ではその歴史,現状,そして将来の展望について述べた。

Active surveillance for adult patients with low-risk papillary thyroid microcarcinoma started at Kuma Hospital in 1993 has been adopted by guidelines conducted by Japan Association of Endocrine Surgery (JAES) and American Thyroid Association. In 2021, consensus statements and position paper have been published from JAES and Japan Thyroid Association, respectively. 要旨
 日本甲状腺学会(JTA)は,甲状腺癌における過剰診断という課題解決の方法として,日本から提唱された。世界的にも受け入れられてきている成人の低リスク甲状腺微小乳頭癌の積極的経過観察(AS)という新しい取扱い方法を含めた微小乳頭癌取扱い方法について広く啓発を図るため,ポジション・ペーパーを制作した。臨床上重要と考えられる課題を疫学,手術成績,診断,ASに分けてCQを設定し,これまでの報告を検討しシステマティックレビューを行った。内容は,甲状腺微小乳頭癌(PTMC)の発生率の増加,低リスクPTMCの良好な手術成績,穿刺吸引細胞診による推奨基準,リンパ節転移(LNM),甲状腺外進展(ETE),遠隔転移の評価に関するエビデンスが含まれる。また,ASについては,やや進行した段階で手術を行ってもその後の再発や生存率への影響がないこと,身体的QOLを維持し,10年間の医療費を削減する可能性が報告されていることから,PTMCの安全で妥当な管理方針ではあるが,適応・実施にあたってはいくつかの注意点があることを記載している

[Running head]The management of low-risk papillary thyroid microcarcinoma in adults

A task force on the management of PTMC in adults organized by the Japan Thyroid Association has published a position paper on how to handle papillary thyroid microcarcinomas(PTMC), including active surveillance of low-risk PTMC in adults, to solve the problem of overdiagnosis in thyroid carcinoma. The task force conducted a systematic review on issues of clinical importance in the areas of epidemiology, surgical outcomes, diagnosis, and active surveillance. The position paper indicates evidence for the increased incidence of PTMC, favorable surgical outcomes for low-risk PTMC, recommended criteria for diagnosis using fi ne needle aspiration cytology, and evaluation of lymph node metastasis(LNM), extrathyroidal extension(ETE)and distant metastasis. Active surveillance has also been reported with a low incidence of disease progression and no subsequent recurrence or adverse events on survival if conversion surgery was performed at a slightly advanced stage. Active surveillance is a safe and valid strategy for PTMC, because it might preserve physical quality of life and reduce 10-year medical costs. The position paper also included some points which should be noted when performing active surveillance.

 近年,甲状腺微小乳頭癌の罹患率増加に伴う過剰診断・過剰治療は,甲状腺癌診療において解決すべき課題の一つとなっている。その解決方法の手段の一つとして甲状腺微小乳頭癌に対する積極的経過観察(AS)が,わが国より発信され世界的に普及してきている。ASは優れた管理方針ではあるものの,適切な適応と方法で実施することで安全に行える方針であり,医療者と患者の理解を深める必要がある。そのため,現在までのエビデンスで,適切と考えられるASの具体的な適応と方法を提示することを目的に,日本内分泌外科学会は患者説明用紙の雛形を含むコンセンサス・ステートメントを制作した。本稿ではコンセンサス・ステートメントのエッセンスを解説し,今後の課題を提示する。本コンセンサス・ステートメントがASの安心で安全な普及,また医療者,患者の納得がいく意思決定の一助となることを望む。

[Running head]Essence and future issues of the management for papillary thyroid microcarcinoma.

In recent years, the overdiagnosis and overtreatment of papillary thyroid microcarcinoma(PTMC)due to the increasing incidence of the disease is one of the issues in thyroid cancer treatment. Active surveillance(AS)of PTMC has been reported in Japan as one of the solutions to this issue and has been gradually accepted worldwide. Although AS is an excellent management strategy, that must be implemented with appropriate indications and methods, and it is necessary to deepen the understanding of medical professionals and patients. For this reason, the Japanese association of endocrine surgery has reported a consensus statement for indications and strategies of AS based on evidence to date, including a model patient information form. This article describes the essence of the consensus statement and suggests future issues. We hope that this consensus statement will contribute to the safe adoption of AS and to improve shared decision-making.

 2019年12月に突如として出現し, 瞬く間に世界中に感染が拡大した新型コロナウイルス感染症(COVID-19)の原因ウイルスは,2002年の重症急性呼吸器症候群(SARS)の原因ウイルスSARS-CoVに類似しており,SARS-CoV-2と名付けられた。これまでに確認されているコロナウイルスのうち,SARS-CoV-1, MERS-CoV,SARS-CoV-2の3つのウイルスは重篤な肺炎を引き起こす。SARSは1年弱で終息し,甲状腺疾患の報告はあったものの十分なデータは集まっていないようである。一方で,2022年1月,本邦ではオミクロン株の流行により第6波が到来し,COVID-19の終息はいまだ不明である。COVID-19患者数の増加に伴い,COVID-19に関連した甲状腺疾患の報告も増加しており,今後その関連性が明らかにされていくだろう。

[Running head]Coronavirus and Thyroid

Coronavirus disease 2019(COVID-19)is a global pandemic caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)that is similar to SARS-CoV-1. We have already identified some coronaviruses and experienced outbreaks of severe coronavirus infection by SARS-CoV-1(SARS)in 2002 and MERS-CoV(MERS)in 2011. SARS ended in less than a year, thus we could not collect sufficient data related to the thyroid diseases. On the other hand, in January 2022, the sixth wave of COVID-19 arrived in Japan due to an outbreak of the Omicron strain. For these past two years, the number of patients with thyroid diseases associated with COVID-19 are increasing.

要旨
 COVID-19はSARS-CoV-2ウイルスの感染によって引き起こされる感染症であり,2019年12月に発生しパンデミックとなった。COVID-19は重大な呼吸器疾患に加え,循環器疾患,凝固異常,自己免疫疾患などの全身合併症をもたらすことが報告されている。COVID-19に関連した甲状腺疾患に関しては,これまで亜急性甲状腺炎,非定型甲状腺炎,Basedow病,橋本病,産後甲状腺炎やH-P-T axisの障害,non-thyroidal illness syndromeが明らかとなった。詳細な病因は明らかでないものの,ウイルス感染後の自己免疫の活性化やSARS-CoV-2による直接の組織障害がそれらの発症に関与している可能性があると考えられている。
 本稿においては,COVID-19と甲状腺疾患について概説し,治療法をはじめとした今後の展開につき検討する。

Coronavirus disease 2019(COVID-19)is the infectious syndrome caused by a novel coronavirus known as severe acute respiratory syndrome coronavirus 2(SARS-CoV-2). The COVID-19 pandemic occurred in December 2019. In addition to the severe respiratory complications, COVID-19 causes abnormalities in cardiovascular and coagulation systems, and autoimmune diseases.
To date, subacute and atypical thyroiditis, Graves’ disease, Hashimoto’s thyroiditis, postpartum thyroiditis, dysfunctions in H-P-T axis, and non-thyroidal illness syndrome, have been reported as a complication of COVID-19.
Although the detailed mechanisms of them remain unknown, activation of autoimmunity during SARS-CoV-2 infection, and virus-mediated direct tissue damage have been proposed to be involved with the development of the thyroid diseases.
In this review, we provide an overview of the thyroid diseases associated with COVID-19, then discuss the potential future directions including treatment for the thyroid diseases.

 Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)感染による新型コロナウイルス感染症(coronavirus disease 2019;COVID-19)は,甲状腺疾患との関連が報告されている。発症時期により,SARSCoV-2感染から比較的短期間に認められる病態(非定型甲状腺炎,非甲状腺疾患など)と,感染から一定期間経過した後に認められる病態(自己免疫性甲状腺疾患,亜急性甲状腺炎など)に大別される。病因論的には,自己免疫性甲状腺疾患に関連する病態とそれ以外の病態に分類される。これらの病態の詳細な発症機構は必ずしも解明されていないが,炎症性サイトカインによる作用,あるいはアンジオテンシン変換酵素2(angiotensin-converting enzyme 2;ACE2)を介したウイルスによる直接の細胞障害などが想定されている。
 本稿では,ウイルス感染症と甲状腺疾患に関するこれまでの知見を振り返るとともに,SARS-CoV-2感染時に想定される病態について概説する。

[Running head]Pathophysiology of COVID-19-induced Thyroid Disorders

Coronavirus disease 2019(COVID-19), caused by severe acute respiratory syndrome coronavirus 2(SARSCoV-2)infection, is characterized by respiratory symptoms. An increase in inflammatory cytokine concentrations is reported to be related to COVID-19 severity. Recent evidence has accumulated for several thyroid disorders associated with COVID-19, including autoimmune thyroid disease, subacute thyroiditis, atypical thyroiditis, non-thyroidal illness, and central hypothyroidism. Research suggests that increased inflammatory cytokine concentrations or cellular invasion of SARS-CoV-2 through angiotensin-converting enzyme 2(ACE2)receptors are responsible in developing COVID-19-related thyroid dysfunction. Th is article presents a literature review on the recent advances in the etiology of COVID-19-related thyroid disorders.

要旨
 新型コロナウイルス感染症(COVID-19)の発症ならびに重症化の予防を目的とし,ワクチン接種が進んでいる。日本においてはBNT162b2,mRNA-1273,ChAdOx1 nCoV-19/AZD1222が薬事承認され,2022年2月16日時点で1回以上の接種者は総人口の80.2%,2回接種完了者は79%,3回接種完了者は11.1%と報告されている(首相官邸ホームページ:https://www.kantei.go.jp/jp/headline/kansensho/vaccine.html)。しかし,ワクチン接種後に起きやすい副反応として,比較的軽度の症状が多い一方で,アナフィラキシーといった重篤なものがまれに起こり得る。また,報告数はきわめて少ないが,ワクチン接種後に亜急性甲状腺炎,Basedow病,無痛性甲状腺炎を発症した症例も報告されている。その機序としてはアジュバントによるautoimmune/inflammatory syndrome induced by adjuvants(ASIA)の他,ワクチンによって産生された中和抗体が甲状腺ペルオキシダーゼ(TPO)に対して交差反応を起こし,自己免疫性甲状腺疾患を誘発する可能性が推測されている。現時点では,COVID-19ワクチン接種と甲状腺疾患発症の因果関係は明らかではないため,引き続き症例の蓄積が望まれる。

COVID-19 vaccinations are progressing to prevent severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection. Vaccines, such as BNT162b2, mRNA-1273 and AZD1222, have been approved in Japan. As of 16 February 2022, 80.2% of the total population had received at least one dose, 79% had received two doses, and 11.1% had received three doses. Although most adverse reactions to vaccines are relatively mild, serious adverse reactions, such as anaphylaxis, may occur. Moreover, a small number of cases with subacute thyroiditis, Graves' disease and painless thyroiditis have been reported after COVID-19 vaccination. It is postulated that, in addition to autoimmune/inflammatory syndrome induced by adjuvants(ASIA), neutralizing antibodies produced by vaccines, which cross-react with thyroid peroxidase(TPO)may lead to the development of autoimmune thyroiditis. At present, the causal relationship between COVID-19 vaccination and the development of autoimmune thyroiditis is not clear, and further investigations are needed.

要旨
 2019年12月に中国において初めての症例が報告されてから,新型コロナウイルス感染症(COVID-19)によるパンデミックは世界中の人々の日常診療に大きな影響を与えてきたが,甲状腺疾患の診療への影響に関するエビデンスは限定的である。当院では2019~2020年にかけて,月あたりの外来受診患者数が大幅に減少し,L-サイロキシン(LT4)を処方され通院している患者ではその減少幅は約20%であった。そのなかでも,高齢・甲状腺刺激ホルモン(TSH)高値・LT4の処方量・処方日数が多い患者ほど,コロナ禍において外来受診の遅れが認められた。さらにコロナ禍における外来受診の遅れは,その後の甲状腺機能のコントロール不良にも関連していた。これらの結果より,コロナ禍であっても,LT4内服患者に対して適切な内服量の確保,血液検査の実施,医療アクセスの保証が重要であると考えられた。さらに,外来受診の遅れに伴い,LT4内服患者だけでなく,甲状腺中毒症患者や甲状腺悪性腫瘍など,甲状腺疾患の悪化が予想される。
 依然収束しないコロナ禍において,(i)長期的な健康アウトカムへの影響,(ii)遠隔診療などの代替手段の有効性についてさらなるエビデンスの蓄積が求められている。

The influence of the COVID-19 pandemic on the clinical management of thyroid diseases

Since the first case of COVID-19 was reported in December 2019, COVID-19 has become a global pandemic. Although the indirect effects of the pandemic on clinical practice have received great attention, evidence regarding management of thyroid disease management associated with COVID-19 is limited. In our recent study using data on >25,000 patients being treated with levothyroxine at Ito Hospital, we found that the number of follow-up visits among patients being treated with levothyroxine has largely decreased since the pandemic.
Patients with a higher dose and a longer duration of levothyroxine prescription before the pandemic were more likely to make a delayed follow-up visit during the pandemic. Patients who made a delayed follow-up visit showed less well-controlled thyroid hormone levels than those who made a follow-up visit as scheduled during the pandemic. Assuring adequate prescriptions, healthcare accessibility, and opportunities for biochemical testing would be critical to managing such patients during and after the COVID-19 pandemic.
In addition to the patients being treated with levothyroxine, those with other thyroid disease(e.g., thyrotoxicosis, thyroid malignancies)may have similar consequences due to the delayed follow-up visit. During the ongoing COVID-19 pandemic, further investigations are needed to evaluate (i)the long-term health outcomes and (ii)effectiveness of virtual care among patients with thyroid diseases.

超音波検査で炎症経過を形態的に観察した甲状腺中毒症を伴う急性化膿性甲状腺炎の1例

要旨
 急性化膿性甲状腺炎に伴う重度甲状腺中毒症を呈する症例を経験し,経時的に超音波検査(US)で炎症過程を形態的に観察した。急性化膿性甲状腺炎の炎症過程をUSで経過観察した報告は少ない。
症例は8歳,女児。発熱,咽頭痛,左前頸部の圧痛と軽度腫脹を主訴に近医小児科を受診。血液検査でCRP高値を認め,当院小児科に紹介入院となった。初診時,前頸部に皮膚発赤を認めず,USでは甲状腺左葉に限局した低エコー腫瘤を認めた。血液検査で白血球増多,CRP異常高値,重度甲状腺中毒症を認めた。抗菌薬投与で一時軽快したが,26日後に皮膚発赤の出現と,USにて低エコー域の皮下への拡大と皮下膿瘍形成を認めた。切開膿瘍ドレナージを施行し,USで低エコー域の縮小を認めた。下咽頭食道造影検査およびUSから,下咽頭梨状窩瘻に起因した急性化膿性甲状腺炎と診断した。甲状腺機能は一過性潜在性機能低下症を経て正常化した。USによる発症初期からの経時的観察が,炎症過程を把握する上で有用であった。

A case of acute suppurative thyroiditis with thyrotoxicosis whose inflammatory process was observed morphologically by serial ultrasonography

[Running head]Ultrasonographic observation of acute suppurative thyroiditis with thyrotoxicosis

A case of severe thyrotoxicosis caused by acute suppurative thyroiditis (AST) was observed morphologically by serial ultrasonography (US). There were only few reports that the inflammatory process was followed up by US. An 8-year-old girl had fever, sore throat and painful swelling of the left anterior neck. Physical examination revealed a 3cm, tender left thyroid mass without skin erythema. Neck ultrasonography (US) revealed a heterogeneous and hypoechoic mass in the left thyroid lobe. The blood assay revealed an increased white blood cell count 20,900/μL and CRP 26mg/dL. Thyroid function test showed severe thyrotoxicosis. The patient was diagnosed with AST and she was subsequently treated with intravenous antibiotics. Skin redness developed 17 days later and US showed an increase of the hypoechoic area in and around the left thyroid lobe. Incisional drainage of neck abscess was performed and dense pus was drained. US showed decrease of the perithyroidal hypoechoic lesion after drainage. Esophagography and US revealed the presence of left pyriform sinus fistula. Thyroid function was restored to normal after transient subclinical hypothyroidism. Serial short-interval US was useful for understanding the inflammatory progression in AST.
